2025-2026 School Registration Forms
The school requires a non-refundable $300 registration fee, payable to St. Francis de Sales School. If registration is completed by February 1st, 2025, this fee will be applied towards tuition as a credit. Pay online by clicking here.
Early registration is encouraged to secure a spot for your child and to assist us in planning for the upcoming school year. If you encounter any challenges or have inquiries during the registration process, do not hesitate to contact the school office at 847-438-7921 or [email protected].
To complete your registration for the 2025-2026 school year, please ensure that the $300 per family registration fee is paid and the following required documentation is submitted to the school office.
Required Documentation (please either bring to the school office or email to [email protected]):
- Birth certificate (All new students)
- Baptismal certificate (All new students)
- Physical Exam Record (Preschool, KG, 2nd, 6th Grade, and all new students)
- Dental Exam Record (KG, 2nd, 6th Grade, and all new students)
- Vision Exam Record (KG and new students)
Once the registration fee and all necessary documentation have been received, the office will send an email confirming your registration for the upcoming school year. Please note that your student(s) will not be considered registered until all items are submitted and the registration fee is paid in full.

Preschool, Kindergarten, & First Grade Age Requirements
Preschool
A child entering the 3-year-old preschool program must be 3 years of age on or before September 1st.
A child entering the 4-year-old and Pre-K program must be 4 years of age on or before September 1st.
Kindergarten
A child entering kindergarten must be 5 years of age on or before September 1st.
Under special circumstances, an underage child may be admitted with Principal approval, but only with preschool recommendation and after all children of proper age have been admitted.
First Grade
A child must be 6 years of age on or before September 1st.
Under special circumstances, an underage child may be admitted with Principal approval, but only with the previous school’s recommendation and after all children of proper age have been admitted.
